deductible or co-pays. This plan pays benefits in addition to any other Insurance. The
issue ages are 18 to 64 years. Dependent child coverage is available to age 19 or 25 if a full time student. You can use any doctor, hospital or licensed provider. Pre-existing
conditions that are incurred within the 12 month period preceding the effective date are covered after 12 Months. Your
rates cannot increase due to your advanced age or declining health. This policy is guaranteed renewable to age 65. Underwritten by Guarantee
Trust Life Insurance Company. 24 Hour Accident Coverage Accident Expense Coverage for Members and their Families.
This plan pays 100% of covered medical expenses after satisfaction of the $100 or $250 deductible up to the coverage maximum of
$2,500, $5,000, $7,500 or $10,000. Benefits are paid directly to you. You can use any doctor or hospital.
